Psychological Protective Factors Mediating the Relationship between Academic Stress and Common Mental Health Problems among Medical Students: A Conceptual Framework

Abstract
Medical students face substantial academic demands arising from intensive coursework, frequent assessments, clinical training, research expectations, and concerns about professional competence. These demands may increase their vulnerability to depression, anxiety, and suicidal ideation. Although academic stress has been consistently associated with mental health problems, the indirect mechanisms linking academic stress to these outcomes have not been sufficiently integrated within a unified theoretical framework. This conceptual paper develops a framework explaining the mediating roles of resilience, social support, and mindfulness in the relationships between academic stress and depression, anxiety, and suicidal ideation among medical students. A focused narrative review and theory-synthesis approach was adopted to integrate relevant stress theories, empirical evidence on medical students’ mental health, and research on personal and social protective resources. Conservation of Resources theory provides the principal theoretical foundation for the framework, conceptualising academic stress as a continuing demand that may threaten or deplete resources required for psychological adaptation. Resilience is conceptualised as an adaptive and recovery resource, social support as an interpersonal and contextual resource, and mindfulness as an attentional and emotional self-regulation resource. The framework proposes positive direct relationships between academic stress and each mental health outcome, together with indirect relationships through the three parallel mediators. Depression, anxiety, and suicidal ideation are treated as separate but potentially correlated outcomes. The framework contributes to the literature by integrating risk and protective perspectives, distinguishing mediation from moderation, and incorporating suicidal ideation alongside depression and anxiety. It also suggests that resource-building initiatives should be accompanied by institutional efforts to address modifiable academic stressors and improve access to mental health support. Longitudinal and culturally sensitive research is required to test the proposed temporal and indirect relationships across medical education contexts.
